Doctor/Reception: Please upload TGA approval and the Script as separate documents in the form below and save them in the following way: If Authorised Prescriber please include your AP number on the script***

2. Script eg John Smith Spectrum YELLOW Script (please include DOB and handwritten as per S8 requirements).

*** Scripts and TGA documents uploaded, emailed or faxed that are not in this format will attract an additional admin fee to the patient. We are trying to keep our dispense fees low so we can pass on the lowest medical cannabis pricing in Australia. Please upload in the format above so your patient will not be charged for additional administration fees. Please post your original Scripts to My Life My Health Pharmacy PO Box 10233, Brisbane Qld 4000. Please don’t give the original script to your patient, please post to the pharmacy.

What we do next:

• On receiving your uploaded documents from this portal, a payment link is emailed to your patient with the price for their Medical Cannabis Medication.
• Once payment is received our dispensary will process your patient’s medication order and notify your patient that their medication is ready via our signature postal service, pickup in store or drive thru. We post Australia wide with a trackable postal service.
• Your patient will receive a series of counselling titration emails that include titration tips, Consumer Medicines Information (CMI) and diet and lifestyle options to enhance their therapy.
• Your patient has access to a Medicinal Cannabis Pharmacist Educator hotline (07) 3832 2888 to help with their Medicinal Cannabis therapy.
